Position: Activity Worker (Full Time)
Hours of Work: 35 hours per week
Reports to: Living Well Manager

Position Summary: Responsible to assist in developing and implementing daily program activities to support established long term and short term rehabilitative and recreational goals based on clients’ level of functioning and to support clients’ independence and safety
Responsibilities: - Assists in developing daily program activities
- Designs, plans and implements appropriate activities for clients based on their physical, mental and cognitive needs and according to the service plan.
- Completes physical s et up of program activities and carries out daily transportation schedule checks and safety checks of program space
- Provides personal care to clients and responds to client emergencies as needed
- Completes administrative duties
- Assists the Team Leader with daily documentation including daily log, daily safety check, client’s attendance, dietary sheet, monthly billing, maintenance job order, monthly program plan, infection rate record and other statistical records in Procura
- Documents clients’ health and functional status or behaviors in health record system
- Promotes health and safety of clients and staff
- Works safely at all times and ensures the safety of self, co-workers and clients by preventing hazards and reporting hazards to the supervisor
- Complies with the organization’s “Client Safety”, “Risk Management” and “Health and Safety”, “People Centered Philosophy” policy statements
- Follows the policy, procedures and handbooks as required by the position, with the emphasis on health and safety for both clients and oneself .
- Performs other support duties
- Ensures service plans are properly implemented by the team
- Participates in team meetings, case discussion and case conferences.
- Ensures the efficient operation of day program by backing up other Day Program staff when absent
Any other reasonable duties that may be assigned from time to time
Education: - Post-secondary education in progress in Nursing, Activision, Gerontology, Pharmacy, Social Services etc.
Qualifications: - Basic understanding of social, recreational or therapeutic programs an asset
- Experience and interest or training in music, crafts, arts or adapted therapeutic activities
- Ability to interact with seniors who are frail, confused or experiencing dementia
- Ability to do physical work such as lifting, pushing wheelchairs, and assisting seniors with sitting, standing and bending
- Ability to speak other languages such as Tamil, Greek, Hindi and Spanish are an asset.
Other: - Vulnerable Sector screening required
- Proof of full COVID-19 vaccination or Medical Exemption certificate required
