ChristianaCare is hiring a full time Community Educator for their School Based Health Programs in New Castle, DE.
As a Community Health Educator you would serve as a front-line staff member, screening for social care needs and outreaching patients to help them navigate community-based resources, connecting patients and families to internal and external resources and services, and providing emotional support as needed. You may also meet patients and their families in their homes and in the communities.
Work Schedule:
Monday-Friday: 8a - 4:30
May travel to Middletown area

Qualifications:
High school diploma required; bachelor's degree preferred.
One-year community and patient experience required, more preferred.
Public speaking, group facilitation and excellent writing skills preferred.
Computer experience required.
Demonstrated ability to work with diverse populations required.
Bi-lingual language skills are a plus.
